GCR 6.5.1.J.3 states (emphasis is mine):
"A car that improves its position relative to the field during the pace lap by moving forward, moving out of line, or passing before the green flag is displayed may be penalized for a false start. However, if a car drops out of the pace lap(s), everyone in the column behind that car must close up behind the car in front; moving up under this circumstance is not considered improving position or passing under yellow."
What this means is that when a car in, say, P4 drops out with a mechanical issue or whatever prior to the start, all the cars in the outside lane advance one position. It does not mean that the car in P5 should move into P4 and so on. If you are gridded in the left-hand column, you start in the left-hand column. We all know this, right?
I've seen the side-switching thing happen a couple of times, and it's a really bad idea.

You are correct, do not move out of line. Just move up. On the same note, anyone moving out of position, ie: spin, mechanical, has given up his starting position and must start at the back of the field.

As you pull on track, the next car will assume the unfilled position and move to the opposite line. The grid person just points the cars to the alternate lanes as they enter the track.
